Billing the Medicare Annual Wellness Visit and a Medical E&M Visit on the Same Day

We still sometimes see physicians perform a medical E&M visit on one day, and schedule a Medicare Annual Wellness Visit (AWV) for another day. Do that if it makes sense for you and your patient – but remember you can do both the same day.  For the AWV, bill CPT code G0439 and DX code Z00.00 or Z00.01; for the medical visit, bill 9921x and the appropriate medical DX code. And most important, append a -25 modifier on the 9921x to indicate these are separate services and you should be paid for both.
